Hello. Today, we have to inform you that as of October 1st, the providers of Vertrees Clinical Group will no longer be providing care at the Cadet Court location in Lebanon, TN.
As a result of this change, we are communicating with you to let you know of the new location that they are going to be practicing from and to arrange for your next appointment at that location. We understand that this is a significant change and may alter how much time it might take to travel to the appointment or the way that you might meet with your provider. Our principal goal is to provide options that will allow you to continue your therapeutic relationship with your provider with little to no interruption.
Your health and well-being are of utmost importance to our staff. We understand that as a result of the change in location, that you may not be able to continue care with Jillian or Thomas. If that is the choice that you make, we will also provide referral locations for you to contact. Please know that we desire to continue to care for all of our patients and have options to be able to do just that.
If you or a family member are currently scheduled in the next two weeks (Oct 2nd thru Oct 13th) at the Cadet Court location, we will be reaching out to you this week to get your appointment adjusted.
